Going West on Elden Street continue under the overpass for the Dulles
Toll Road. Take your second right at Sunrise Valley Drive and immediate
left on Dulles Technology Drive. The clinic is located the first blue roofed
building in the Connections for Hope Center-- on the right hand side as
you enter
Going West on 267- Dulles Toll Road, take Elden Street exit for Herndon.
Take a left at the exit onto Centerville Rd. Take your second right onto
Sunrise Valley Drive and immediate left on Dulles Technology Drive. The

Children are seen for well and sick
visits Monday and Tues 9:00 -5:00
p.m. and Wed. 9:00 - 12:00 p.m.
Evening clinics are for school and
sports physicals only. Appointments
are recommended but not
required. Parents must provide
proof of income (pay stubs or copies
of tax returns) to qualify for service.
Adult Services
The clinic serves adults with high
blood pressure or diabetes who live
in Herndon, Reston, Centreville, and
Chantilly. Enrollment applications
for adults are accepted Thursday 9-4
p.m. and appointments are
recommended. Patients must bring
photo identification and proof of
income.
